Global Cancer Deaths to Double by 2030 by Salynn Boyles

Cancer deaths are projected to more than double worldwide over the next two decades, largely from a dramatic increase in cancer incidence in low- and middle-income countries driven by tobacco use and increasingly Westernized lifestyles.

A new report from the International Agency for Research on Cancer (IARC) explores the global burden of cancer, which is poised to become the leading cause of death worldwide by 2010. The report predicts that by 2030, 27 million new cancer cases and 17 million cancer deaths will occur each year worldwide. That compares to 12 million new cancers and slightly less than 8 million cancer deaths in 2007.

People really need to stop smoking. And we are pretty lame a society when we inflict such needless disease and death on our fellow humans. Curing millions of cancer patients 20 years from now will be very hard. but it isn’t hard to “cure” millions of them today. We just need people not to pay a lot of money to give themselves cancer by smoking.

Do Breast Tumors go Away on Their Own?

Authors of a new study hope to begin a debate challenging the conventional wisdom about early detection of breast cancer. In an article in today’s Archives of Internal Medicine, they ask: Do breast tumors ever go away on their own?

Researchers of this controversial article note that one type of cancer found through screening — a rare childhood tumor, called neuroblastoma — sometimes disappears. In the new article, researchers try to learn if the same phenomenon occurs with invasive breast cancers found with mammograms

The Natural History of Invasive Breast Cancers Detected by Screening Mammography

Conclusions: Because the cumulative incidence among controls never reached that of the screened group, it appears that some breast cancers detected by repeated mammographic screening would not persist to be detectable by a single mammogram at the end of 6 years. This raises the possibility that the natural course of some screen-detected invasive breast cancers is to spontaneously regress.

As with so much medical research the results are not completely clear. Studies need to be followed by more studies, which often lead to more studies. As long as progress is being made this is a perfectly reasonable course of scientific inquiry. And even if progress is not being made this can be perfectly reasonable – finding answers can be hard.

Rate of Cancer Detected and Death Rates Declines

Declines in Cancer Incidence and Death Rates in report from the National Cancer Institute and CDC:

“The drop in incidence seen in this year’s Annual Report is something we’ve been waiting to see for a long time,” said Otis W. Brawley, M.D., chief medical officer of the American Cancer Society (ACS). “However, we have to be somewhat cautious about how we interpret it, because changes in incidence can be caused not only by reductions in risk factors for cancer, but also by changes in screening practices. Regardless, the continuing drop in mortality is evidence once again of real progress made against cancer, reflecting real gains in prevention, early detection, and treatment.”

According to a U.S. Surgeon General’s report, cigarette smoking accounts for approximately 30 percent of all cancer deaths, with lung cancer accounting for 80 percent of the smoking-attributable cancer deaths. Other cancers caused by smoking include cancers of the oral cavity, pharynx, larynx, esophagus, stomach, bladder, pancreas, liver, kidney, and uterine cervix and myeloid leukemia.

Diagnoses Of Cancer Decline

The analysis found that the overall incidence of cancer began inching down in 1999, but not until the data for 2005 were analyzed was it clear that a long-term decline was underway. “The take-home message is that many of the things we’ve been telling people to do to be healthy have finally reached the point where we can say that they are working,” Brawley said. “These things are really starting to pay off.”

Brawley and others cautioned, however, that part of the reduction could be the result of fewer people getting screened for prostate and breast cancers. In addition, the rates at which many other types of cancer are being diagnosed are still increasing

Some experts said the drop was not surprising, noting that it was primarily the result of a fall in lung cancer because of declines in smoking that occurred decades ago. They criticized the ongoing focus on detecting and treating cancer and called for more focus on prevention.

“The whole cancer establishment has been focused on treatment, which has not been terribly productive,” said John C. Bailar III, who studies cancer trends at the National Academy of Sciences. “I think what people should conclude from this is we ought to be putting most of our resources where we know there has been progress, almost in spite of what we’ve done, and stop this single-minded focus on treatment.”

Nobel Prize in Physiology or Medicine 2008

The Nobel Prize in Physiology or Medicine for 2008 with one half to Harald zur Hausen for his discovery of “human papilloma viruses causing cervical cancer” and the other half jointly to Françoise Barré-Sinoussi and Luc Montagnier for their discovery of “human immunodeficiency virus.”

Harald zur Hausen went against current dogma and postulated that oncogenic human papilloma virus (HPV) caused cervical cancer, the second most common cancer among women. He realized that HPV-DNA could exist in a non-productive state in the tumours, and should be detectable by specific searches for viral DNA. He found HPV to be a heterogeneous family of viruses. Only some HPV types cause cancer. His discovery has led to characterization of the natural history of HPV infection, an understanding of mechanisms of HPV-induced carcinogenesis and the development of prophylactic vaccines against HPV acquisition.

Françoise Barré-Sinoussi and Luc Montagnier discovered human immunodeficiency virus (HIV). Virus production was identified in lymphocytes from patients with enlarged lymph nodes in early stages of acquired immunodeficiency, and in blood from patients with late stage disease. They characterized this retrovirus as the first known human lentivirus based on its morphological, biochemical and immunological properties. HIV impaired the immune system because of massive virus replication and cell damage to lymphocytes. The discovery was one prerequisite for the current understanding of the biology of the disease and its antiretroviral treatment.

Black Raspberries Slow Cancer by Alter Hundreds of Genes

Researchers at the Ohio State University Comprehensive Cancer Center examined the effect of freeze-dried black raspberries on genes altered by a chemical carcinogen in an animal model of esophageal cancer

“We have clearly shown that berries, which contain a variety of anticancer compounds, have a genome-wide effect on the expression of genes involved in cancer development,” says principal investigator Gary D. Stoner

Stoner notes that black raspberries have vitamins, minerals, phenols and phytosterols, many of which individually are known to prevent cancer in animals. “Freeze drying the berries concentrates these elements about ten times, giving us a power pack of chemoprevention agents that can influence the different signaling pathways that are deregulated in cancer,” he says.

Their analyses included measuring the activity, or expression levels, of 41,000 genes. In the carcinogen-treated animals, 2,261 of these genes showed changes in activity of 50 percent or higher.

Pretty cool stuff.

Using Spice-based Compound To Kill Cancer Cells

Synthetic molecules, derived from curcumin, a naturally occurring compound found in the spice turmeric have been killed cancer cells, in lab settings. Centuries of anecdotal evidence and recent scientific research suggest curcumin has multiple disease-fighting features, including anti-tumor properties. However, when eaten, curcumin is not absorbed well by the body. Instead, most ingested curcumin in food or supplement form remains in the gastrointestinal system and is eliminated before it is able to enter the bloodstream or tissues.

James Fuchs, assistant professor of medicinal chemistry and pharmacognosy at Ohio State University and principal investigator on the project, and colleagues are continuing to refine compounds that are best structured to interact with a few overactive proteins that are associated with cell activity in breast and prostate cancers. Blocking these molecular targets can initiate cell death or stop cell migration in the cancers.

A major component of their strategy is called structure-based, computer-aided design, a relatively new technology in the drug discovery field. Before ever working with an actual compound, the scientists can make manipulations to computer-designed molecules and observe simulated interactions between molecules and proteins to predict which structural changes will make the most sense to pursue.

“Most of the interaction between our compound and the overactive protein comes from what are called hot spots on the protein’s surface,” said Chenglong Li, assistant professor of medicinal chemistry and pharmacognosy at Ohio State and an expert in computational chemistry. “For each spot, we can design small chemical fragments and link them together to make a molecule. This is what computer-aided design and modeling can do.”

Some of the most effective compounds have been tested for their effectiveness against human cancer cell lines – as well as whether they might be toxic to healthy cells. So far, the molecule favored by the researchers has a nearly 100-fold difference in toxicity to cancer cells vs. healthy cells, meaning it takes 100 times more of the compound to kill a healthy cell than it does to kill a cancer cell.

Cloned Immune Cells Clear Patient’s Cancer

Cloned immune cells cleared patient’s cancer

A patient whose skin cancer had spread throughout his body has been given the all-clear after being injected with billions of his own immune cells. Tests revealed that the 52-year-old man’s tumours, which spread from his skin to his lung and groin, vanished within two months of having the treatment, and had not returned two years later.

Doctors attempted the experimental therapy as part of a clinical trial after the man’s cancer failed to respond to conventional treatments.

The man is the first to benefit from the new technique, which uses cloning to produce billions of copies of a patient’s immune cells. When they are injected into the body they attack the cancer and force it into remission.

There are many more wonderful announcements than wonderful solutions that live up to the hope provided by the announcement. Still this is one in the long line of potentially wonderful treatments. If it turns out to be successful the whole world will benefit which is an example of why I am thankful so many countries are investing in science and technology.

2008 Intel Science Talent Search

When Shivani Sud was six years old, one of her immediate family members was diagnosed with a brain tumor. Inspired by the doctors who helped her loved one recover, Shivani set her sights on a career in medicine.

Fast forward to Intel STS 2008, where the 17-year-old senior from Durham, North Carolina, presented research focused on identifying stage II colon cancer patients at high risk for recurrence and the best therapeutic agents for treating their tumors. Whereas the standard method of characterizing tumors relies on visual information (such as size, degree
of metastasis, and microscopic structure), Shivani developed a “50-gene model” which uses gene expression to link multiple genetic events that characterize various tumor types to more accurately predict the recurrence of colon cancer. Additionally, Shivani’s model can be used to identify drugs that may be most effective in treating each patient.

In 2006 she attended the Davidson Institute, summer program at Davidson College and received the Davidson Fellows Scholarship.

And now I can segue to the Davidson Wildcats victories in the NCAA basketball tournament last weekend (I graduated from Davidson, by the way, and grew up in Madison). Davidson will play the Wisconsin Badgers on Friday. My picks are doing pretty well.

Cheap, ‘safe’ drug kills most cancers

It sounds almost too good to be true: a cheap and simple drug that kills almost all cancers by switching off their “immortality”. The drug, dichloroacetate (DCA), has already been used for years to treat rare metabolic disorders and so is known to be relatively safe. It also has no patent, meaning it could be manufactured for a fraction of the cost of newly developed drugs.

Evangelos Michelakis of the University of Alberta in Edmonton, Canada, and his colleagues tested DCA on human cells cultured outside the body and found that it killed lung, breast and brain cancer cells, but not healthy cells. Tumours in rats deliberately infected with human cancer also shrank drastically when they were fed DCA-laced water for several weeks.

DCA attacks a unique feature of cancer cells: the fact that they make their energy throughout the main body of the cell, rather than in distinct organelles called mitochondria. This process, called glycolysis, is inefficient and uses up vast amounts of sugar.

Until now it had been assumed that cancer cells used glycolysis because their mitochondria were irreparably damaged. However, Michelakis’s experiments prove this is not the case, because DCA reawakened the mitochondria in cancer cells. The cells then withered and died

The University of Alberta is raising funds to further the research. Some look at this and indite a funding system that does not support research for human health unless there is profit to be made. Much of the blame seems to go to profit focused drug companies. I can see room for some criticism. But really I think the criticism is misplaced.

The organizations for which curing cancer is the partial aim (rather than making money) say government (partial aim or public health…), public universities (partial aim of science research or medical research…), foundations, cancer societies, private universities… should fund such efforts, if they have merit. Universities have huge research budgets. Unfortunately many see profit as their objective and research as the means to the objective (based on their actions not their claims). These entities with supposedly noble purposes are the entities I blame most, not profit focused companies (though yes, if they claim an aim of health care they I would blame them too).

Now I don’t know what category this particular research falls into. Extremely promising or a decent risk that might work just like hundreds or thousands of other possibilities. But lets look at several possibilities. Cancer Deaths – Declining Trend?

3,000 fewer cancer deaths:

Cancer deaths in the United States dropped for the second year in a row, health officials reported yesterday, confirming that the trend is real and becoming more pronounced, too.

The news was cause for celebration among doctors and politicians. “When we saw the first decline, the number wasn’t that enormous,” Dr. Felice Schnoll-Sussman, a cancer physician at New York-Presbyterian Hospital/Weill Cornell Medical Center said. “But once you start to see a trend like this, it obviously makes you feel like ‘We must be doing something right! ‘”

Is this really a trend? I have not examined the data at all but I seriously doubt it. People (the media even more so) constantly overreact to variation in data. Maybe I am wrong, certainly I should look at the data and see what it says – and I will if I get some time and remember. But I am more confident in my belief this is more overreaction to random variation than in the headlines. Why? Because so often when I do look more closely at the numbers my general observation of overreaction to random variation is confirmed while news reports talk of “trends.” Hopefully I am wrong this time.
