Are you ready for a world without antibiotics?

[Professor Tim Walsh] “This is potentially the end. There are no antibiotics in the pipeline that have activity against NDM 1-producing enterobacteriaceae. We have a bleak window of maybe 10 years, where we are going to have to use the antibiotics we have very wisely, but also grapple with the reality that we have nothing to treat these infections with.”

And this is the optimistic view – based on the assumption that drug companies can and will get moving on discovering new antibiotics to throw at the bacterial enemy. Since the 1990s, when pharma found itself twisting and turning down blind alleys, it has not shown a great deal of enthusiasm for difficult antibiotic research. And besides, because, unlike with heart medicines, people take the drugs for a week rather than life, and because resistance means the drugs become useless after a while, there is just not much money in it.

“The emergence of antibiotic resistance is the most eloquent example of Darwin’s principle of evolution that there ever was,” says Livermore. “It is a war of attrition. It is naive to think we can win.”

I have been writing about the huge risks we are talking with our future for years. The careless misuse of antibiotics is very costly (in human lives, in the future). Bacteria pose great risks to us. We need to take antibiotics to fight serious threats. The misuse of antibiotics by doctors, patients, agri-business… is the problem. And we are all living a much riskier future because far to little is being done to reduce the misuse of antibiotics.

More and more antibiotic treatments are losing effectiveness as bacteria evolve resistance. The evolution is accelerated by misuse. This costs lives today, but is likely to costs many thousands and hundreds of thousands and possible more in the next 50 years.

The NDM-1-producing bacteria were highly resistant to all antibiotics except tigecycline and colistin. In some cases, isolates were resistant to all antibiotics. The emergence of NDM-1 positive bacteria is potentially a serious global public health problem as there are few new anti-Gram-negative antibiotics in development and none that are effective against NDM-1.

CDC Urges Reduction in Salt Intake to Save Hundreds of Thousands of Lives

Most people know we eat far too much salt and that it is killing lots of us. It is still amazing that we have over 100,000 people in the USA every year die this way and yet we barely pay attention. Doesn’t it seem like we should care more about life?

Excessive dietary sodium consumption increases blood pressure, which increases the risk for stroke, coronary heart disease, heart failure, and renal disease. Based on predictive modeling of the health benefits of reduced salt intake on blood pressure, a population-wide reduction in sodium of 1,200 mg/day would reduce the annual number of new cases of coronary heart disease by 60,000—120,000 cases and stroke by 32,000—66,000 cases.

Fewer than 10% of all adults in the USA met their recommended limit. U.S. adults consumed an average of 3,466 mg/day of sodium. Most of the daily sodium consumed came from grains (1,288 mg; 36.9%) and meats, poultry, fish, and mixtures (994 mg; 27.9%).

In the United States, an estimated 77% of dietary sodium intake comes from processed and restaurant foods and approximately 10% comes from table salt and cooking. More details from the CDC.

Vaccines Can’t Provide Miraculous Results if We Don’t Take Them

Vaccine preventable diseases used to ravage our health. In the USA, we are lucky to live in a society where those before us have taken vaccines and reduced to very low levels the attack vectors for these diseases. If nearly everyone is vaccinated for polio, even if it crops up with one person, most likely it won’t spread. As more people chose to risk the health of others in the society by failing to vaccinate, an infection can spread rapidly. There are some people who can’t be vaccinated for one reason or another (normally dangerous allergies) and vaccines, while very effective are not 100% effective. So any person that fails to vaccinate their kids endangers society and those who cannot be vaccinated.

Six Top Vaccine Myths

Myth 1: It’s not necessary to vaccinate kids against diseases that have been largely eradicated in the United States.
Reality: Although some diseases like polio and diphtheria aren’t often seen in America (in large part because of the success of the vaccination efforts), they can be quite common in other parts of the world. The Centers for Disease Control and Prevention warns that travelers can unknowingly bring these diseases into the United States, and if we were not protected by vaccinations, these diseases could quickly spread throughout the population. At the same time, the relatively few cases currently in the U.S. could very quickly become tens or hundreds of thousands of cases without the protection we get from vaccines. Brown warns that these diseases haven’t disappeared, “they are merely smoldering under the surface.”

Most parents do follow government recommendations: U.S. national immunization rates are high, ranging from 85 percent to 93 percent, depending on the vaccine, according to the CDC.

We should not delude ourselves into thinking that those diseases are not dangerous. They are. We have been protected by all those taking vaccines. If people in the society don’t take vaccines that increases the health risks to the society at large.

Routine smallpox vaccination among the American public stopped in 1972 after the disease was eradicated in the United States. The United States government has enough vaccine to vaccinate every person in the United States in the event of a smallpox emergency (mainly due to concerns about bio-terrorism).

U.S. Adults Dying of Preventable Diseases

Diseases easily preventable by adult vaccines kill more Americans each year than car wrecks, breast cancer, or AIDS.

“We have a chronic disease epidemic in the U.S. It is taxing our families and taxing our economy,” the CDC’s Anne Schuchat, MD, said at the news conference. “We have a need for culture change in America. We worry about things when they are really bad rather than focusing on prevention, which can keep us out of the hospital and keep our families thriving.”

In other parts of the world the danger is not from those who chose not to vaccinate their children but those who are not provided the opportunity to.

Students Will Spend Year Doing Career-Changing Research Thanks to HHMI

This year, 116 medical, dental, and veterinary students from 47 schools across the country will take a break from memorizing molecular metabolism and studying drug interactions to spend a year in a lab doing hands-on research. The break from regular coursework, funded through a $4 million Howard Hughes Medical Institute (HHMI) initiative, is intended to give students an opportunity to immerse themselves in science and consider whether they want to pursue a career as a physician-scientist.

Nearly 500 medical students applied for the research year through the HHMI-National Institutes of Health (NIH) Medical Research Scholars and HHMI Medical Research Fellows programs. Both efforts seek to strengthen and expand the pool of medically-trained researchers. The funding HHMI provides is a great resource.

“We want medical, dental, and veterinary students to become immersed in the life of academic science for at least a year. And we hope they get so engaged in the process and life of scientific research that they will decide to continue it for the rest of their lives,” says Peter Bruns, HHMI’s vice president for grants and special programs. “We need more doctors who do basic research to improve human health.”

As part of its commitment to fostering the translation of basic research discoveries into improved diagnoses and treatments, HHMI has developed a range of programs to nurture the careers of researchers who bridge the gap between clinical medicine and basic science. In addition to the programs for medical students, the Institute supports medical training for Ph.D. students in the basic sciences and has made specific efforts to fund top physician-scientists as HHMI investigators.

The medical research scholars and fellows programs are open to medical, dental, and veterinary students enrolled in U.S. schools. Most have completed the second or third year of their professional program when they spend a year working in a lab either at the NIH or at an academic medical center or research university they select. During the last 25 years, more than 2,100 students have participated.

The HHMI Medical Research Fellowships program allows medical, dental, and veterinary students to pursue biomedical research at a laboratory anywhere in the United States except the NIH campus in Bethesda. Each student submits a research plan to work in a specific lab with a mentor they have identified. Since 1989, about 1,200 students have participated.

This year, 74 students from 26 medical schools and two veterinary schools were chosen as fellows from a pool of 274. While most students elect to stay at their home institution to do their research, this year 17 fellows will work in labs at a different school. Their research topics include schizophrenia, wound healing, organ development, and many other important biological questions.

The HHMI-NIH Research Scholars program was established in 1985 to encourage medical students to pursue research by allowing them to take a year off from their medical studies. The program has since been expanded to include dental and veterinary students. It has enabled about 1,000 students to work in NIH labs.

Students selected as research scholars often enter the program with only a general idea of what type of research they would like to do. As soon as they are accepted, they begin researching the more than 1,100 laboratories at NIH. They meet with a number potential mentors before finalizing which project to pursue under the guidance of their NIH advisor and HHMI’s staff. The students are sometimes called “cloister scholars” because they live in apartments or dorm-style rooms in a refurbished cloister on the NIH campus in Bethesda.

This year, 42 students from 28 medical schools and one veterinary school were chosen as research scholars. More than 200 students from 93 schools applied.

Food Rules: An Eater’s Manual

Good advice from author Michael Pollan on eating from his new book, Food Rules: An Eater’s Manual. Essentially he suggests eating food. Stuff you can picture in the original form (apples, cashews, celery, trout, tomatoes, grapes, steak, strawberries, milk, figs, peppers, peaches, almonds, chicken) not chemical additions (yes I know real food is made up of chemical – this is additional chemicals). One quote: “the biggest gains in human health can be made from changes in food policy.”

Human health is a complex topic but if we care about our health it is a tough issue we have to try to understand. He makes a good point in his talk about the value of exercise. I do believe exercise is an important component to how to be healthy (as is food – I don’t think it is easy to be healthy without both).

Norway Reduces Infections by Reducing Antibiotic Use

Norway conquers infections by cutting use of antibiotics

Twenty-five years ago, Norwegians were also losing their lives to this bacteria. But Norway’s public health system fought back with an aggressive program that made it the most infection-free country in the world. A key part of that program was cutting back severely on the use of antibiotics.

Now a spate of new studies from around the world prove that Norway’s model can be replicated with extraordinary success, and public health experts are saying these deaths — 19,000 in the U.S. each year alone, more than from AIDS — are unnecessary.

“It’s a very sad situation that in some places so many are dying from this, because we have shown here in Norway that Methicillin-resistant Staphylococcus aureus [MRSA] can be controlled, and with not too much effort,” said Jan Hendrik-Binder, Oslo’s MRSA medical advisor. “But you have to take it seriously, you have to give it attention and you must not give up.”

The World Health Organization says antibiotic resistance is one of the leading public health threats on the planet. A six-month investigation by The Associated Press found overuse and misuse of medicines has led to mutations in once curable diseases like tuberculosis and malaria, making them harder and in some cases impossible to treat.

Now, in Norway’s simple solution, there’s a glimmer of hope.

Re-engineering the Food System for Better Health

According to the Centers for Disease Control, between 1980 and 2006 the percentage of obese teenagers in the United States grew from 5 to 18, while the percentage of pre-teens suffering from obesity increased from 7 to 17.

Obesity is widespread due to our national-scale system of food production and distribution, which surrounds children – especially lower-income children – with high-calorie products…
90 percent of American food is processed – according to the United States Department of Agriculture – meaning it has been mixed with ingredients, often acting as preservatives, that can make food fattening.

Now, in another report finished this October after meetings with food-industry leaders, the MIT and Columbia researchers propose a solution: America should increase its regional food consumption.

Only 1 to 2 percent of all food consumed in the United States today is locally produced. But the MIT and Columbia team, which includes urban planners and architects, believes widespread adoption of some modest projects could change that, by increasing regional food production and distribution.

To help production, the group advocates widespread adoption of small-scale innovations such as “lawn to farm” conversions in urban and suburban areas, and the “10 x 10 project,” an effort to develop vegetable plots in schools and community centers. Lawns require more equipment, labor and fuel than industrial farming nationwide, yet produce no goods. But many vegetables, including lettuce, cucumbers and peppers, can be grown efficiently in small plots.

As Albright sees it, the effort to produce healthier foods “fits right in with the health-care reform effort right now because chronic diseases are so costly for the nation.” America currently spends $14 billion annually treating childhood obesity, and $147 billion treating all forms of obesity.

Good stuff. We need to improve health in the USA. The current system is unhealthy and needs to be improved. The public good from improving the health of society is huge (both in terms of individual happiness and economic benefits).

The Only Known Cancerless Animal

Unlike any other mammal, naked mole rate communities consist of queens and workers more reminiscent of bees than rodents. Naked mole rats can live up to 30 years, which is exceptionally long for a small rodent. Despite large numbers of naked mole-rats under observation, there has never been a single recorded case of a mole rat contracting cancer, says Gorbunova. Adding to their mystery is the fact that mole rats appear to age very little until the very end of their lives.

The mole rat’s cells express p16, a gene that makes the cells “claustrophobic,” stopping the cells’ proliferation when too many of them crowd together, cutting off runaway growth before it can start. The effect of p16 is so pronounced that when researchers mutated the cells to induce a tumor, the cells’ growth barely changed, whereas regular mouse cells became fully cancerous.

“It’s very early to speculate about the implications, but if the effect of p16 can be simulated in humans we might have a way to halt cancer before it starts.” says Vera Gorbunova, associate professor of biology at the University of Rochester and lead investigator on the discovery.

In 2006, Gorbunova discovered that telomerase—an enzyme that can lengthen the lives of cells, but can also increase the rate of cancer—is highly active in small rodents, but not in large ones.

Until Gorbunova and Seluanov’s research, the prevailing wisdom had assumed that an animal that lived as long as we humans do needed to suppress telomerase activity to guard against cancer. Telomerase helps cells reproduce, and cancer is essentially runaway cellular reproduction, so an animal living for 70 years has a lot of chances for its cells to mutate into cancer, says Gorbunova. A mouse’s life expectancy is shortened by other factors in nature, such as predation, so it was thought the mouse could afford the slim cancer risk to benefit from telomerase’s ability to speed healing.

While the findings were a surprise, they revealed another question: What about small animals like the common grey squirrel that live for 24 years or more? With telomerase fully active over such a long period, why isn’t cancer rampant in these creatures?

How to Stay Healthy: Avoiding the Flu

It is no secret that washing your hands is a great strategy to stay healthy. Still few people take care to wash their hands thoroughly frequently during each day. The H1N1 Flu is just the latest sickness that washing your hands protects you from.

The CDC recommends you take these everyday steps to protect your health to protect yourself from the flu:

  • Wash your hands often with soap and water, especially after you cough or sneeze. Alcohol-based hand cleaners are also effective. Though the scientific evidence is not as extensive as that on hand washing and alcohol-based sanitizers, other hand sanitizers that do not contain alcohol may be useful for killing flu germs on hands.
  • Avoid touching your eyes, nose or mouth. Germs spread this way.
  • Try to avoid close contact with sick people.
  • And to keep others healthy you should

  • Cover your nose and mouth with a tissue when you cough or sneeze. Throw the tissue in the trash after you use it. [others also suggest coughing into you elbow instead of your hands, again to reduce the spread of germs.]
  • If you are sick with flu-like illness, CDC recommends that you stay home for at least 24 hours after your fever is gone except to get medical care or for other necessities. (Your fever should be gone without the use of a fever-reducing medicine.) Keep away from others as much as possible to keep from making others sick.

The spread of this 2009 H1N1 flu is thought to be happening in the same way that seasonal flu spreads. Flu viruses are spread mainly from person to person through coughing or sneezing of people with influenza. Sometimes people may become infected by touching something with flu viruses on it and then touching their mouth or nose.

The incidence of H1N1flu is likely to be high this flu season based on results in the Southern Hemishpere. Symptoms are those of the flu: fever, cough, sore throat, runny or stuffy nose, body aches, headache, chills and fatigue; can include diarrhea and vomiting.

The CDC includes weekly flu statistics on their web site. Since mid-April to August 30, 2009, a total of 9,079 hospitalizations and 593 deaths associated with 2009 influenza A (H1N1) viruses have been reported to CDC an increase from 8,843 hospitalizations and 556 deaths from the prior week.

How a software engineer tried to save his sister and invented a breakthrough medical device

I wanted to help my sister as much as I could. I went to Medline, where there are hundreds of thousands of documents describing clinical studies, to see what I could find.

There are billions of dollars spent every year on clinical studies. I was surprised to discover that there were sometimes clinical studies of treatments for which there were no clinical applications. The trials would show successful results but no clinical applications.

I found a 1987 Italian funded set of clinical studies that showed successful treatment of tumors by the application of chemotherapy directly into the tumors. But I could find nothing since then.

It took us two years to do the engineering. And it has taken the FDA seven years and two months to approve the product for sale. We were able to shorten the FDA process a little by saying that it was similar to other devices that had already been approved.

Great stuff.
